ABSTRACT

Objective To explore the roles of different insulin resistance indexes[triglyceride-glucose (TyG),triglyceride (TG)/high-density lipoprotein cholesterol (HDL-C),and metabolic score for insulin resistance (METS-IR)]and combinations of two indexes in predicting diabetes risk in hypertensive population. Methods The survey of hypertension was conducted for the residents in Wuyuan county,Jiangxi province from March to August in 2018.The basic information of hypertensive residents was collected by interview.Blood was drawn on an empty stomach in the morning and physical measurements were carried out.Logistic regression model was employed to analyze the relationship between different insulin resistance indexes and diabetes,and the area under the receiver operating characteristic curve was used for evaluating the predictive effects of each index on diabetes risk. Results A total of 14 222 hypertensive patients with an average age of (63.8±9.4) years old were included in this study,including 2616 diabetic patients.The diabetic hypertensive population had higher TyG (t=50.323,P<0.001),TG/HDL-C (Z=17.325,P<0.001),and METS-IR (t=28.839,P<0.001) than the non-diabetic hypertensive population.Multivariate analysis showed that each insulin resistance index was positively correlated with diabetes risk.The area under curve of each insulin index was in a descending order of TyG (0.770)> METS-IR (0.673)> TG/HDL-C (0.620).The difference in the area under curve between two indexes was statistically significant[TyG vs.TG/HDL-C (Z=42.325,P<0.001);TyG vs.METS-IR(Z=17.517,P<0.001);METS-IR vs.TG/HDL-C (Z=10.502,P<0.001)]. Conclusions Elevated insulin resistance indexes can increase the risk of diabetes.TyG and the combination of indexes outperform TG/HDL-C and METS-IR in the prediction of diabetes.

ABSTRACT

Objective:To study the interaction effects between intestinal microflora features with serum inflammatory factors and homeostatic model assessment insulin resistance (HOMA-IR) in patients with first-episode untreated schizophrenia (SCZ).Methods:A total of 79 SCZ patients admitted to Suining Central Hospital from August 2016 to August 2019 were selected as observation group, and 60 healthy examiners were set as control group. The intestinal microflora, serum inflammatory factors and glucose metabolism indexes of the two groups were detected and compared. The correlation between intestinal microflora and inflammatory factors and HOMA-IR was analyzed.Results:The content of bifidobacteria in the observation group was lower than that in the control group while the content of bacteroides was higher than that in control group; the levels of serum inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and IL-6] and in the observation group were higher than those in the control group ( P<0.05); the level of HOMA-IR in the observation group was higher than that in control group ( P<0.05). The TNF-α, IL-1β, IL-6 and HOMA-IR in SCZ patients were significantly negatively correlated with intestinal bifidobacteria content ( P<0.05), and were obviously positively correlated with bacteroides content ( P<0.05). Conclusions:In first-episode untreated SCZ, the content of bifidobacteria decreased and the content of bacteroides increased, and there was a significant correlation with inflammatory factors and HOMA-IR, which may lead to the disorder of glucose and lipid metabolism and induce the occurrence of metabolic diseases such as diabetes.

ABSTRACT

Objective:To investigate the correlation between multiple parameters of islet function evaluation and atherogenic index (AIP) in patients with type 2 diabetes mellitus (T2DM).Methods:A cross-sectional study was conducted to observe 216 T2DM patients hospitalized in Tianjin Medical University Chu Hsien-I Memorial Hospital in 2019. They were divided into non atherogenic phenotype group (N Group, AIP<0.06) and atherogenic phenotype group (A group, AIP≥0.06) with AIP=0.06 as the cut-off point. The general clinical indexes and evaluation indexes of oral glucose tolerance test (OGTT), such as insulin action index (IAI), quantitative insulin sensitivity check index (QUICKI), homeostasis model of insulin resistance index (HOMA-IR), homeostasis model assessment-β (HOMA-β), C peptide insulin resistance index (HOMA IR-CP) and C peptide islet function index suitable to diabetes patients (HOMA islet-CP DM) were compared between two groups. Pearson correlation analysis and multiple stepwise regression analysis were performed.Results:Compared with the N group, the very low density lipoprotein-cholesterol (VLDL-C), ln (HOMA IR-CP) and ln (HOMA islet-CP DM) in A group were higher ( P<0.01), while ln (QUICKI) was lower ( P<0.05), with statistical significance. AIP was positively correlated with VLDL-C ( r=0.765), ln (HOMA-IR) ( r=0.257), ln (HOMA-β) ( r=0.189), ln (HOMA IR-CP) ( r=0.418) and ln (HOMA islet-CP DM) ( r=0.377, P<0.01), and negatively correlated with IAI ( r=-0.145, P<0.05) and ln (QUICKI) ( r=-0.254, P<0.01). Multiple stepwise regression analysis showed that VLDL-C and ln (HOMA IR-CP) were independent influencing factors of AIP in type 2 diabetic patients ( P<0.01) . Conclusions:AIP is positively correlated with insulin resistance and negatively correlated with insulin sensitivity. VLDL-C and HOMA IR-CP are independent predictors of AIP.

ABSTRACT

Objective To investigate the effect of electroacupuncture on hypothalamic insulin receptor substrate 1 (IRS-1) in a rat model of type 2 diabetes mellitus (T2DM). Methods Sixty Wistar rats were randomized to a normal group (15 rats) and an observation group (45 rats). In the observation group, a rat model of T2DM was made by high-energy diet induction. After the model was successfully made 8 weeks later, the observation group was randomized to model making, treatment and blocker groups, 15 rats each. The treatment group received electroacupuncture and the blocker group, electroacupuncture plus intraventricular perfusion of phosphatidylinositol 3-hydroxyl kinase (PI3K) blocker. After 8 weeks of treatment, fasting plasma glucose (FPG) was measured using a glucometer, fasting insulin (Fins) was determined by ELISA, insulin resistance index (IRI) was calculated and IRS-1 expression was examined by SABC immunohistochemistry assay in every group of rats. Results FPG and Fins increased significantly (both P<0.01) and IRI and IRS-1 expression decreased significantly (P<0.01) in the model making group compared with the normal group. FPG and Fins decreased significantly (both P<0.01) and IRI and IRS-1 expression increased significantly (P<0.01) in the treatment group compared with the model making group. FPG and Fins decreased significantly (P<0.05, P<0.01) and IRI and IRS-1 expression increased significantly (P<0.01) in the treatment group compared with the blocker group. Conclusion Electroacupuncture can improve FPG, Fins and insulin sensitivity by regulating hypothalamic IRS-1 expression in T2DM rats.

ABSTRACT

Objective To investigate the characteristics and significance of inflammatory factors and insulin resistance index in patients with type 2 diabetes mellitus complicated with depression.MethodsThe patients with type 2 diabetes mellitus complicated with depression were treated with IL-2, TNF-α and HOMA-IR in control group.The patients in type 2 diabetes mellitus had no complication and depression., Record the above indicators of test results.ResultsIL-2, TNF-α and HOMA-IR were the highest in the study group (P<0.05).The levels of IL-2, TNF-α and HOMA-IR in control group 1 and control group 2 were significantly higher than those in control group (P<0.05).There was no significant difference between the two groups.ConclusionThere is a high level of insulin resistance index and inflammatory factor in patients with type 2 diabetes mellitus complicated with depression.Therefore, it is suggested that the treatment of type 2 diabetes mellitus can be treated with anti-inflammatory and insulin resistance.The.

ABSTRACT

Objective To compare the clinical characteristics of pregnant women with gestational diabetes mellitus (GDM) and normal gestational glucose metabolism at the 25th weeks of pregnant. Methods Sixty-eighty patients with GDM (GDM group) and 68 patients with normal blood glucose(control group) were enrolled in this study. During 25 weeks of pregnancy, oral glucose tolerance test (OGTT), blood pressure, fasting insulin, glycosylated hemoglobin, uric acid, triglyceride, total cholesterol, high density lipoprotein cholesterol and low density lipoprotein cholesterol were measured and body mass index (BMI), homeostasis model of assessment for insulin resistence index (HOMA-IR), insulin sensitivity index (ISI) were computed. The results were compared between two groups. Results The age in GDM group was significantly higher than that in control group: (31.38 ± 0.54) years vs. (29.50 ± 0.56) years, P0.05), but diastolic blood pressure in GDM group was significantly higher than that in control group:(73.2 ± 0.8) mmHg vs. (70.9 ± 0.8) mmHg, 1 mmHg=0.133 kPa, P0.05). Conclusions Compared to those with normal blood glucose, the patients with GDM have the characteristics of higher age, higher body weight, higher BMI, higher diastolic blood pressure , higher level of insulin, insulin resistance and decreased insulin sensitivity.

ABSTRACT

Objective To assess the agreement between homeostasis model assessment for insulin resistance index(HOMA-IR),a simple index of fasting serum triglyceride(TG)and plasma glucose(TyG index)and fasting plasma glucose(FPG)/fasting insulin(FIns)with glucose disposal rate(GDR)in patients with newly diagnosed type 2 diabetes(T2DM). Methods A total of 137 patients with newly diagnosed T2DM were enrolled.The levels of FPG,FIns,TG and GDR were measured in baseline. Agreement analysis was performed to assess their agreements. Results The bias and limit of agreement of HOMA-IR,FPG/FIns and TyG index with GDR were - 3.776(- 7.495 ~ - 0.055),- 8.428 (-25.311~ 8.456)and 1.075(- 2.598 ~ 4.749),respectively,and their intra-class correlation coefficients(ICC)were 0.797,0.503 and 0.236,respectively. Conclusion HOMA-IR,TyG index and FPG/FIns showed the acceptable agreement with the GDR. The poor reliability of FPG/FIns and TyG index(ICC<0.6)limits its clinical application.

ABSTRACT

Objective To explore the association between type 2 diabetes mellitus (T2DM)combined with non -alcoholic fatty liver disease (NAFLD)and serum adiponectin (APN)and its clinical significance.Methods A total of 76 patients who were initially diagnosed with T2DM or had been diagnosed with T2DM for less than three years and admitted to the endocrine department and its outpatient clinic of the Affiliated Hospital of Medical School of Ningbo University from August 2011 to July 2013 were selected as the subjects,and were assigned to group A or B,each consisting of 38 patients,based on the presence or absence of associated NAFLD.Another 30 healthy subjects receiving physical examination were assigned to the control group (NC group).The indicators including body mass index (BMI),blood lipids,blood glucose,liver function,insulin resistance index (HOMA -IR)and serum APN,were measured and compared among the groups. Results In group A ,the indicators including BMI,triglyceride (TG),total cholesterol (TC),low density lipoprotein -cholesterol (LDL -C),alanine aminotransferase (ALT),fasting insulin (FINS)and HOMA -IR were significantly higher than those in group B and NC group,and the APN level was significantly lower than that in group B and NC group (P <0.05).Pearson correlation analysis indicated that APN was negatively correlated with BMI,TG,high density lipoprotein -cholesterol(HDL -C),and HOMA -IR,and positively correlated with TC,LDL -C and ALT(P <0.05).Multiple linear regression analysis suggested that HDL -C and HOMA -IR was the independent influencing factors for APN (P <0.05).Conclusion Severe insulin resistance and glucose and lipid metabolic disorders are present in T2DM patients combined with NAFLD,and adiponectin level is of vital significance for the development and progression of NAFLD.

ABSTRACT

Objective To investigate the effects of Wuling Powder on insulin resistance of C57BL/6J mice induced by high lipid diet, and discuss the mechanism. Methods C57BL/6J mice were randomly divided into six groups:normal group, model group, rosiglitazone group, Wuling Powder in low, middle, and high dose groups, 10 mice per group. Besides the normal group, other five groups were fed with high fat and sugar diet, with a purpose to establish insulin resistance model. Normal group and model group were given pure water. Rosiglitazone group received a gavage with rosiglitazone of 0.75 mg/kg. Wuling Power low, middle, and high groups received gavage with Wuling Power of 1.23, 3.69, 11.07 g/kg, respectively, the does volume was 0.2 mL/10 g, once a day. The weight and abdominal girth were detected every week. At the end of the sixth week, mice were given 12-hour fasting, and their eyeball were taken for blood. The body weight, length, and fat in abdomen and both kidneys were detected. Paraffin section was made with HE staining. FPG and FINS of each group were detected. ISI and IRI were calculated, and TC and TG were detected. Results Compared with the model group, Wuling Powder can significantly reduce the body weight and abdominal girth of mice (P<0.01, P<0.001), improve liver fatty degeneration, lower the FPG, FINS, TCH, TG, IRI, and increase the ISI in mice (P<0.05, P<0.01). Conclusions Wuling Powder has the effect of preventing insulin resistance of C57BL/6J mice induced by high lipid diet.

ABSTRACT

Objective To evaluate the serum 25-hydroxy vitamin D 3 level in newly diagnosed type 2 diabe-tes,and its relationship with function of islet beta cell .Methods 60 newly diagnosed type 2 diabetes patients and 48 normal glucose tolerance healthy control cases were selected .The level of serum 25-hydroxy vitamin D3 of two groups were measured using the way of high performance liquid chromatograph .The difference was compared between two groups.Insulin resistance index(HOMA-IR) and islet beta cell secrete index(HOMA-β) was used to respectively es-timate insulin sensitivity and islet beta cell function in type 2 diabetes group .The correlation was analyzed between 25-hydroxy vitamin D 3 and fasting blood glucose ,HOMA-IR,HOMA-β.Results The level of serum 25-hydroxy vita-min D3 (28.68 ±1.61) ng/mL in type 2 diabetes group was significantly lower than that of control group (41.30 ± 1.12)ng/mL(t=3.47,P0.05).Conclusion The level of 25-hydroxy vitamin D 3 was significantly low in newly diagnosed type 2 diabetes patients .Low serum 25-hydroxyl vitamin D 3 level affects islet beta secretion cell function ,the supplementation of vitamin D 3 may be a simple and effective method to reduce the ocurrence of type 2 diabetes .

ABSTRACT

Objective To investigate the differences of leptin (LEP) between Mongol and Han population with metabolic syndrome (MS) and its related factors.Methods According to the diagnostic criterion of MS,291 people with MS were selected as subjects,of which,146 were Han nationality(A group) and 145 were Mongol(B group).Radioimmunoassay kit was used to measure the serum leptin level.At the same time,the indices including weight,height,blood pressure,blood glucose,blood lipid,Serum uric acid (sUA),insulin (Fins),body mass index (BMI) and insulin resistance index (HOMA-IR) were measured.Results The following indices in B group including fasting plasma glucose (FPG),Low-density lipoprotein cholesterol (LDL-C),leptin,blood insulin,insulin resistance index were (6.2 ± 1.5) mmol/L,(3.1 ± 0.8) mmol/L,(4.3 ± 2.0) μg/L,(22.4-± 16.0) mU/L and (6.5 ± 0.5) respectively,significantly differed from that of A group ((6.7 ±1.7) mmol/L,(2.7 ±0.7) mmol/L,(3.4 ± 1.5) μg/L,(18.8 ±14.0) mU/L,(4.7 ±3.6)respectively;t =2.04,2.84,3.47,2.18,4.82 ;P < 0.01 or P < 0.05).There was no significant difference in terms of age((46.9 ±9.8) vs.(46.3 ± 8.4)),systolic blood pressure (SBP) ((146.8 ± 17.0) mm Hg vs.(149.1 ±19.2) mm Hg),diastolic blood pressure (DBP) ((90.5 ± 11.6) mm Hg vs.(92.5 ± 13.1) mm Hg),BMI ((27.4 ± 2.9) kg/m2 vs.(27.9 ± 3.2) kg/m2),total cholesterol (TC) ((5.5 ± 1.0) mmol/L vs.(5.5 ±0.9) mmol/L),triacylglycerol (TG) ((2.3 ± 1.4) mmol/L vs.(2.3 ± 1.4) mmol/L),high density lipid cholesterol (HDL-C) ((1.3 ±0.3) mmol/L vs.(1.2 ±0.4) mmol/L),and sUA (((320.7 ±93.6)μmol/L) vs.(308.7 ±86.9) μmol/L) between the patients with metabolic syndrome in Mongol population and in Han population(t =0.47,0.90,1.15,1.15,0,0,1.00,0.94 respectively,P > 0.05).The increase of leptin level in the patients with metabolic syndrome in B group was associated with blood glucose,blood insulin and insulin resistance index (r =0.108,0.146,0.183 ; P < 0.05).BMI,blood insulin and insulin resistance index may be the factors due to the higher of serum leptin levels.Conclusion The serum leptin of patients with metabolic syndrome in Mongol population are correlated with blood glucose,blood insulin and insulin resistance index,which plays an important role in the development of metabolic syndrome.

ABSTRACT

Objective To investigate children with nephrotic syndrome (NS) receiving long-term glucocorticoid treatment while monitoring insulin resistance(IR) index,and analyze the influencing factors,and to observe the efficacy of metformin for preventing glucocorticoid-induced obesity and diabetes mellitus type 2.Methods A total of 32 cases including 20 boys and 12 girls with mean age of 7.6 years old(2 to 14 years old),were diagnosed as NS.All children undergoing long-term treatment of prednisone > 1 mg/(kg · d) for at least 3 months.The height,body weight and body mass index(BMI) were monitored.The concentrations of serum insulin and C-peptide were determined by radioimmunoassay,the concentrations of serum HbA1 c were determined by high performance liquid chromatography,and the levels of fasting glucose,cholesterol and triglyceride were measured with an automatic measuring analyzer (American ABBO TT CCX-Ⅱ).The homeostasis model assessment-estimated insulin resistance(HOMA-IR) was calculated;When HOMA-IR > 3.5,the patients were given metformin treatment for 12 weeks while supervising their height,weight,fasting blood glucose levels monthly.Results Out of 32 cases of NS with long-term use of prednisone,23 cases had HOMAIR > 3.5 (72%).Thus,IR group included 23 patients,and non-IR (NIR) group had 9 patients.There were significant differences between 2 groups in BMI,age,prednisone dosage and the levels of triglyceride (TG) (all P < 0.05).After 12 weeks of metformin treatment,the HOMA-IR (5.24 ± 1.82 vs 2.54 ± 1.09,P < 0.01),HbA1 c levels [(6.36 ±0.82)% vs(5.39 ±0.51)%,P <0.05],BMI[(27.42 ±6.12) kg/m2 vs(22.72 ±5.48) kg/m2,P <0.01],and TG levels[(2.03 ± 1.10) mmol/L vs(1.45 ±0.48) mmol/L,P<0.05] were significantly reduced.Conclusions Older children with long-term use of glucocorticoid may easily lead to IR.Therefore,monitoring the HOMA-IR and early administration of mefformin is helpful for prevention and treatment of glucocorticoid-induced obesity and diabetes mellitus type 2.

ABSTRACT

Objective To investigate the relationship and the influence between pre-diabetes mellitus (PDM) and hyperuricemia (HUA).Methods 157 PDM patients,aged 20 to 75 years old were selected from the Second Clinical Medical College of Harbin Medical University,from 2009 February to 2010 February and were divided into HUA group (76 cases) and NUA group (81 cases).All the patients had not been on thiazide drugs.T-test and Pearson correlation analysis were used to calculate the differences and correlation between uric acid and biochemical indicators.Results In the HUA group,BMI was (27.74 ± 2.88) kg/m2,waist to height ratio (WSR) was (0.55 ± 0.41),TC was (6.61 ± 0.73) mmol/L,TG was (3.94 ± 1.97) mmol/L,LDL-C was (3.60 ± 0.45) mmol/L and homeostasis model assessment-insulin resistance index (HOMA-IR) was (3.09± 1.20).There were significant differences noticed in BMI,TG,TC,LDL-C,HOMA-IR at higher level in the HUA group than those in the NUA group.Pre-diabetes uric acid levels were positively correlated with TG,TC,LDL-C while HOMA-1R (TG:r=0.29,TC:r=0.33,LDL-C:r=0.49,HOMA-IR:r=0.51,P<0.05)was negatively correlated (r=-0.30,P<0.05) with the HbAlc.Conclusion The levels of PDM uric acid might both be related with TC,TG,LDL-C and HOMA-IR.The High level of uric acid status in vivo appeared closely related to HOMA-IR,which could further promote the progress of pre-diabetic patients to diabetes and causing dyslipidemia.Our findings suggested that the levels of pre-diabetes uric acid levels should be under concern.

ABSTRACT

ObjectiveTo compare the sensitivity and specificity of a new index of insulin resistance (IR) derived from plasma triglyceride and glucose with homeostasis model assessment for insulin resistance index ( HOMA-IR) and insulin sensitivity index from frequent sampling intravenous glucose tolerance test (FSIGT-ISI).Methods A total of 1024 subjects (240 normal control,335 with obesity,312 impaired glucose regulation,and 137 type 2 diabetes mellitus) were included in the present study.Standard oral glucose tolerance test,insulin release test,lipid profiles,and other biochemical markers were measured. Among them 540 subjects were selected to undertake FSIGT.TyG index is derived from plasma triglyceride and fasting glucose.TyG2 index is derived from plasma triglyceride and postprandial 2 plasma glucose.ResultsPearson correlation coefficient between TyG and HOMA-IR or FSIGT-ISI was 0.427 ( P<0.01 ) and -0.100 ( P=0.024 ),respectively,and that between TyG2 and HOMA-IR or FSIGT-ISI was 0.455 ( P<0.01 ) and - 0.162 ( P<0.01 ),respectively.The sensitivity and specificity of TyG index for diagnosis of IR was 68.5% and 63.5% compared with HOMA-IR,and 68.5% and49.5% compared with FSIGT-ISI.For TyG2 index,the slightly higher sensitivity ( 81.7% with HOMA-IR,75.7% with FSIGT-ISI),but lower specificity (51.5% with HOMA-IR,48.2% with FSIGT-ISI) were found.ConclusionsBothTyGandTyG2 indices could be used as a surrogate for assessing IR in Chinese subjects.However,considering its moderately high sensitivity but low specificity,these two indices are limited in the use of large-scale epidemiological screening.

Objective To study the correlation between serum adiponectin along with insulin resistanee and vascular dilation function in patients with different rates of glucose metabolism.Method A total of 83 patients aged 50-65 years,including 17 with normal glucose tolerance(NGT),22 with impaired fasting glucose (IFG),23 with impaired glucose tolerance(IGT)and 21 with diabetes(DM)were enrolled. Serum adiponectin was measured,and homeostasis model assessment for insulin resistance index (HOMA-IR)and B cell function index were used to evaluate the function of insulin secretion before and during oral glucose tolerance test(OGTr).The brachial artery responses to flow-mediated endothelium-dependent vascular dilation(EDVD)and nitroglycerin-mediated vascular dilation (EIVD) were evaluated by using the vascular ultrasound with high resolution.Results The serum adiponectin and EDVD decreased that the degrees of reduction from slightness to greatness were in turn from NGT,IFG,IGT to DM,whereas,the insulin resistance index (HOMA-IR) increased just in reverse order.Adiponectin was negatively associated with HOMA-IR(r=-0.353,P0.05).In a multiple regression analysis with a stepwise manner to predict adiponectin concentration and EDVD.HOMA-IR was supposed to predict adiponectin concentration.Meanwhile APN but not HOMA-IR was used to predict EDVD(P<0.05).Condusions The decrease in serum adiponectin,endothelial dysfunction and insulin resistance can be observed in the early stage of impaired glucose metabolism. Serum adiponectin concentration appears to be associated with vascular function and insulin resistance.The association between serum adiponectin concentration and vascular function seems to be independence from its link with insulin resistance index.

ABSTRACT

Purslane extract in the form of ethanolic formulation is rich in polyphenols, flavonoids and anthocyanin, w-3 fatty acids and melatonin. The present study was designed to investigate the anti-obesity and anti-diabetic effects of purslane using obese diabetic rats. The rats received either regular diet, high-fat diet or high-fat diet with additional purslane (150 and 300 mg/kg body weight) for 8 weeks. Purslane, co-administered with a high fat diet, significantly inhibited body weight gain, blood glucose, triglyceride, total cholesterol, LDL-C, HDL-C, free fatty acids and the atherogenic index levels in a dose dependent manner. Purslane-treated rats at doses of 150 and 300 mg/kg body weight improved the insulin resistance index when compared to high fat diet control. In conclusion, purslane ethanolic extract showed effects indicative of potential anti-obesity and anti-diabetic actions in rats fed a high fat obesity-induced diet.

@#ObjectiveTo investigate the role of serine/threonine kinase (Akt) and lipoprotein lipase (Lpl) in formation of nonalcoholic steatohepatitis (NASH) in rat with a fat-rich diet.Methods40 male Wistar rats were randomly divided into the 8-week-rich-fat group (n=10), 8-week-common-diet control group (n=10), 12-week-rich-fat group (n=10) and 12-week-common-diet control group (n=10). The lipid, cholesterol, glucose and insulin were examined, insulin resistance index (IRI) and insulin sensitivity index (ISI) were calculated, the expression of Akt and Lpl were detected with immunohistochemical method and morphological changes of liver were observed by transmission electron microscope (TEM).ResultsNASH was formed in each rat of 8-week-rich-fat group with characteristics of individual obese, increased liver volume, full and clear contour, gray-yellowish luster, greasy section and crisp quality. And it was accompanied by high lipoidemia (HL), liver fat cell denaturation, inflammatory cell infiltration in liver lobuler and cell death in liver. The expression of Akt was obviously reduced and the expression of Lpl was obviously weakened in liver. Lipid, cholesterol, glucose and insulin and IRI were gradually advancing. ISI was reducing and the insulin resistance formed. The data of fat-rich diet groups was significantly different with that of common-diet control groups (P<0.0001).Conclusion Steatohepatitis and insulin resistance can form in rat by feeding with rich-fat diet for 8 weeks. It causes the insulin and Lpl activeness reducing, and steatolysis barrier.

ABSTRACT

PURPOSE: Childhood Obesity is increasing throughout the world, and it is known to incur many diseases especially in later life such as diabetes and cardiovascular disorders. The purpose of this study was to investigate the association between obesity and insulin, insulin-like growth factor-I (IGF-I), insulin-like growth factor binding protein-3 (IGFBP-3) and know if these factors are useful in predicting cardiovascular diseases. METHODS: The study group consisted of 64 moderate and severe obese adolescents and the controls were normal adolescents of the same age. body mass index (BMI) was calculated by height and weight; systolic and diastolic blood pressure was measured at resting state. After 10-hour fasting period, blood cholesterol, triglyceride, high density lipoprotein (HDL) cholesterol, low density lipoprotein (LDL) cholesterol, glucose, insulin, free fatty acid, IGF-I and IGFBP-3 were measured. RESULTS: Insulin was significantly higher in the obese adolescent group than the control group (obese group 15.6+/-7.0 microIU/mL, P< 0.01). IGF-I was also significantly higher in the obese adolescent group than the control group (obese group 498.1+/-122.2 ng/mL, P< 0.05). In addition, IGFBP-3 was significantly higher in the obese adolescent group than the control group (obese group 3, 777+/-4, 721 ng/mL, P< 0.05). Insulin showed significantly positive correlation with BMI (r=0.3944) and obesity index (r= 0.34). IGFBP-3 were significantly correlated with obesity index (r=0.419), diastolic blood pressure (r=0.264) and BMI (r=0.247). Insulin resistance index significantly positive correlation with BMI (r= 0.595), blood triglycerid level (r=0.515) and obesity index (r=0.469). CONCLUSION: Serum insulin, insulin resistance index, IGF-I and IGFBP-3 levels may be useful to predict cardiovascular diseases in adolescent obesity.

ABSTRACT

Objective To compare the saccharometabolism with the pancreatic islets functions and insulin resistance index in children with severe stress. Methods Thirty children with severe stress and 30 healthy children in control group were tested. The levels of fasting blood glucose (FBG), fasting insulin (FINS) and fasting C - peptide (FCP) were detected by radioimmunoassay respectively and insulin sensitivity index (ISI), insulin resistance index (IR) and fasting blood cell function index (FBCI) were calculated statistically. Results There were significant differences between the children with severe stress and the normal controls in the levels of FINS, FCP and FBG,(all P0.05). Conclusion There is insulin resistance with the significant decrease in the insulin sensitivity index and significant increase in insulin resistance index in the children with severe stress, which may cause the disorder in glucose metabolism in children with severe stress.
